How to Get CE Credit

To receive credit for this CE activity, you must complete the online educational activity as well as an CE Activity Evaluation and Credit Request Form, and score 70% or higher on the posttest.

Online:

To receive CE credit, you must view the online presentations, close the open window to

RETURN to the CE Information page. Scroll to the bottom, click the link to access

Posttest, complete test, evaluation form and CRF and claim your CE credit. Then print your certificate online. That's it—no need to wait for the certificate to be mailed to you!

Fax/US Mail:

To receive credit by mail, you must view the presentation, print out the .pdf of the posttest, credit request and evaluation forms, and complete all information. Forms must be submitted prior to the expiration date of the activity to: The FCG Institute for Continuing Education <insert project number>, 1140 Welsh Road, Suite 210, North Wales, PA 19454; or Fax to 215-412-9686. A certificate will be mailed within 4 weeks of our receipt of the completed form to each participant who successfully completes this activity. There is no fee to participate in this CE activity.
